隋开皇十年（590年），以梁信郡改置（1）。领梁信、封兴、都城、威城、晋化、苍梧、封阳等县。郡治梁信县，治所即今广东省封开县封川镇。辖境相当今广东省封开县南部和郁南县西部境域，以及广西梧州市和贺县南部地区（2）。大业三年（607年），改为苍梧郡（3）。

（1），《隋书》卷31?地理下?苍梧郡：“梁置成州，开皇初改为封州。”封川县：“梁曰梁信，置梁信郡。平陈，郡废。十八年改为封川。大业初又废封兴县入焉。都城，开皇十二年省威城、晋化二县入焉。苍梧，旧置苍梧郡。封阳。”（北京，中华书局，1973年，3册，883页）。
（2），据所领县故址及其后政区变迁和《一统志》广东广西各县沿革，辖境相当今广东省封开县南部和郁南县西部境域，以及广西梧州市和贺县南部地区。
（3），《旧唐书》卷41?岭南道?封州：“隋苍梧郡之封川县。”领县四：封川，州所治，梁置梁信郡。隋平陈，改为成州，又改为封州。隋末，州废为封川县，属苍梧郡。隋移州于封川口，即今县治。（北京，中华书局，1975年，5册，1719页）。",
