宋文帝元嘉二十年（443年）置。领乐昌，始昌，乐山等县。郡治乐昌县，治所即今广东省四会县北地豆镇。辖境相当今广东省四会县东北部地区和三水县西北部地区。齐建元元年（479年），迁治始昌县。

（1），《宋书》卷38?州郡志四?广州刺史：“乐昌郡：乐昌令，始昌令，宋元令，乐山令，义立令，安乐令。”（北京，中华书局，1974年，4册，1203-1204页）。按：《宋书》广州所领诸郡，均书有置立年份，唯乐昌郡不书；又文帝元嘉中期，广州立宋康、绥建、海昌、宋熙诸郡，今作文帝元嘉二十年置乐昌郡，设乐昌县为郡治。
（2），据所领县故址，辖境相当今广东省四会县东北部地区和三水县西北部地区。
（3），《读史方舆纪要》卷101?肇庆府?高要县?乐昌废县：“在县北。刘宋置县，为乐昌郡治。齐移郡治始昌，以乐昌县并入。隋郡废，又并始昌等县入四会县。…”（北京，中华书局，1955年，5册，4201页）。姑定点于县北地豆。
《嘉庆一统志》卷447?肇庆府?古迹?始昌废县：“在四会县北。宋置乐昌郡，治乐昌县。领始昌，宋元，乐山，安乐等县。齐省乐昌，移郡治始昌，仍领三县，增置义立县。隋平陈郡废，大业初并省入四会。”（《四部丛刊续编》，商务印书馆，1934年，167册）。",
