梁天监六年（507年）置。郡治梁乐县，治所即今广东省阳山县南杨梅镇。辖境相当今广东省阳山县南部地区。隋开皇十年（590年）废属连州。

（1），《隋书》卷31?地理下?熙平郡：“平陈，置连州。统县九。桂阳，梁置阳山郡，平陈郡废，大业初置熙平郡。阳山。连山，梁置，曰广德，隋改曰广济，仁寿元年改名焉。宣乐，梁置，曰乐梁并置乐梁郡，平陈郡废，十八年改为宣乐。游安。熙平，旧置齐乐郡，平陈郡废。武化，梁置。桂岭，旧曰兴安，开皇十八年改名焉。开建，梁置南靖郡，平陈郡废。”（北京，中华书局，1973年，3册，896页）。《舆地纪胜》卷92?连州阳山县下云，“梁天监六年置阳山郡，以县属焉。”据此，梁乐郡同作梁天监六年置。
