梁天监六年（507年）置（1）。领含洭、阳山、桂阳等县。郡治含洭县，治所即今广东省英德县含洸镇。辖境相当今广东省连县，连南，连山，阳山等县和英德县西部地区（2）。隋开皇十年（590年），改为洭州（3）。

（1），《隋书》卷31?熙平郡：“平陈，置连州，统县九：桂阳，梁置阳山郡，平陈郡废，大业初置熙平郡。”（北京，中华书局，1973年，3册，896页）。《舆地纪胜》卷92?连州阳山县下，作“梁天监六年置阳山郡。”
《隋书》卷31?地理下?南海郡：“旧置广州、梁、陈并置督府。平陈，置总管府。仁寿元年置番州，大业初府废。统县十五。…。含洭：梁置衡州、阳山郡。平陈，州改曰洭州，郡废。二十年州废。”（北京，中华书局，1973年，3册，880-881页）。
